Nesiritide telah diperkenalkan di AS sebagai rawatan intravena baharu untuk pesakit dengan kegagalan jantung kongestif dekompensasi akut yang mengalami dyspnea semasa rehat atau dengan aktiviti yang minimum ity. Nesiritide ialah bentuk rekombinan manusia pertama bagi peptida natriuretik jenis B vasodilator yang kuat (juga dikenali sebagai peptida natriuretik otak), 32 peptida asid amino yang wujud secara semula jadi dengan struktur cincin 17 asid amino terikat disulfida. Nesiritide mengikat kepada reseptor peptida natriuretik Atype pada permukaan endothelial dan sel otot licin yang merangsang pengeluaran cGMP utusan kedua yang menjadi pengantara terutamanya kelonggaran sel otot licin vaskular. Ia juga memudahkan penyingkiran natrium dan air oleh buah pinggang dan mengurangkan rembesan hormon tertentu, seperti adrenalin, angiotensin II, aldosteron dan endothelin, yang menimbulkan kesan buruk jangka panjang termasuk penyempitan saluran darah dan peningkatan tekanan darah. Dalam ujian klinikal dengan pesakit kegagalan jantung, bergantung kepada dos nesidtide mengurangkan tekanan baji kapilari pulmonari, tekanan atrium kanan dan rintangan vaskular sistemik dan peningkatan indeks jantung tanpa menjejaskan kadar denyutan jantung. Kesan buruk utama yang diperhatikan ialah hipotensi yang bergantung kepada dos. Dalam percubaan perbandingan fasa III, nesiritide didapati lebih baik daripada iv. nitrogliserin dalam kesan hemodinamiknya serta lebih mudah untuk ditadbir dan diterima dengan lebih baik. Nesiritide dibersihkan melalui pembelahan proteolitik oleh enzim neutral endopeptidase NEP24.11 dan dengan mengikat kepada reseptor peptida natriuretik jenis C diikuti oleh endositosis dan hidrolisis lisosom intraselular. Memandangkan ia mempunyai separuh hayat yang pendek (18 min), nesiritide ditadbir sebagai infusi bolus 21\u03bcg\/kg diikuti dengan infusi penyelenggaraan berterusan pada 0.011\u03bcg\/kg\/min biasanya dalam tempoh 24-48 jam.<\/p>\n
